Donald R. Dvorak

Donald R. Dvorak, age 57 of Three Lakes, WI died on May 5, 2014 at the Seasons of Life Hospice House in Woodruff, WI. Donald was born on June 16, 1956 in Chicago, IL the son of Arthur and Annette (nee Adamczyk) Dvorak. Donald was raised in Niles, IL and moved to the Northwoods in 1997. He loved snowmobiling, boating, and the outdoors. He was an avid Nascar fan and loved working on cars and motorcycles. He is survived by his Children: Ryan Dvorak and Brandi Dvorak both of Arlington Heights, IL and by his Sister, Cynthia Boyle of Three Lakes, WI and Brothers, James of Port St. Lucie, FL and Gregory of Arlington Heights, IL and further survived by nieces, nephews and other relatives.
